Idiopathic palpitations in silent cardiac phenomena: implications for Primary Care.

Palpitations are among the most frequent reasons for cardiovascular consultation in Primary Care. Although they follow a benign course in the majority of cases, in a small subgroup they may correspond to persistent and silent arrhythmic phenomena, with significant functional and emotional impact. Their paroxysmal and unpredictable nature, together with initially normal complementary test results, makes early diagnosis challenging.
This case illustrates that persistent palpitations, even in the initial absence of structural abnormalities, may correspond to clinically relevant arrhythmias. Primary Care plays a key role in longitudinal follow-up, integration of clinical information, and detection of subtle evolutionary patterns. Periodic reassessment and coordinated care are essential to avoid diagnostic delays, reduce the associated emotional burden, and optimize patient safety and quality of life.
